Forms

The following Forms are available for contractors:

• Claim Form-Please fill this form out, then both Submit and Print the form.  The completed form will be sent to the Indigent Defense Services automatically.  HOWEVER please note further processing of your claim can not be done until you submit all required supporting documentation to the Indigent Defense Services.
• Administrative Request For Funding
• Request for Additional Compensation-Please complete and submit this form if you reasonably anticipate exceeding 25 hours on a particular case.  Ensure that you submit this form prior to exceeding 25 hours of work on the case.  Remember to track your hours and submit all hours worked on the case to include the first 25 so that your claims may be processed promptly.
• Authorization for Electronic Payment- Please fill out this form, print it, sign it and return it to IDS to begin to receive your payments by means of electronic transfer to your selected account.  This will expedite the payment of your claims and save you the hassle of having to manually deposit paper checks.
• Credit Memo- Please utilize this form to issue a memo of credit to Mohave County Indigent Defense Services. 

The following Forms are available for internal agency employees only:

• PDO Notice of Intent Form- This form is used to request funding for expert or professional services by employees of the Public Defender's Office only. 
• LDO Notice of Intent Form- This form is used to request funding for expert or professional services by employees of the Legal Defender's Office only. 
• LAO Notice of Intent Form- This form is used to request funding for expert or professional services by employees of the Legal Advocate's Office only.
