Vital Records

The Department of Public Health is also responsible for the vital record functions in Mohave County related to processing birth and death certificates and issuing certified copies to the public upon completed application and proper acceptable identification documentation as required by the Arizona Department of Health Services.


Mohave County processes applications for certified birth and death records, amendments, corrections, home birth, and special fetal & infant certificates via drop-off, or through U.S. mail. If choosing the mail option, please include a self-addressed, stamped envelope.
