Birth Certificates

Who Can Obtain a Birth Certificate

Only persons 18 years of age or older may obtain a certified copy of a birth certificate.

Arizona Vital Records has birth records ONLY for births that occurred in Arizona.
If you need a certified copy of a birth certificate from another state visit the CDC for more information.

Note: An applicant presenting documents from a foreign country as proof of relationship, or proof of legal interest, must provide certified copies of the documents. Photocopies are not acceptable. The applicant must also provide a certified translation of the documents.
 
Arizona is a "closed record" state. That means that vital records are not public record. Arizona law restricts the public's access to vital records as follows to protect the confidentiality rights of our citizens. Arizona Administrative Code (A.A.C.) R9-19-210 and R9-19-211 limits who can receive a certified copy of a birth certificate.
Instructions for Applying by Dropbox or by Mail 

Applications Submitted via Dropbox

Each location has a secured dropbox. When submitting an application via dropbox, please ensure you have the following:

  • A copy of your valid government-issued picture ID which bears your signature.
  • Proof of eligibility. More information at AZDHS
  • Appropriate fee
  • If possible, complete and print the Application Form and bring it with you. (A copy of the application is also available in the Vital Records office.)

Applications Submitted Through the Mail

  • A completed Application Form.
  • A copy of your valid government-issued picture ID which has your signature. Or, have your signature notarized.
  • Proof of eligibility. More information at AZDHS.
  • Appropriate fee

Certified copies are $20.00 per copy and we accept debit/credit card, money order, cashiers check or exact cash. We DO NOT accept personal checks.


As of 1/1/2019, the state FEE for birth records are:

Certified Birth Certificate..................................$20.00
Certified Delayed Birth Certificate....................$20.00
Amendment of Certified Birth Certificate..........$30.00
Non-certified Copy............................................$ 5.00

*NOTE* Fee for corrections or amendments is $30 which includes one (1) revised copy of the certified birth certificate.
 
Acceptable and Non-Acceptable Identification for Requesting Birth Records
 
Acceptable:
 
• United States issued Drivers Licenses
• United States issued Identification Card
• United States Territories Drivers License or Identification Card
• Tribal Identification Cards that contains bearer’s signature
• U.S. Military ID Cards that contains bearer’s signature (some do not)
• Passports-United States or Foreign issued
• Visa –In a passport and the bearer’s signature is on the passport not the Visa
• Foreign Consulate Identification Card
• Resident Alien Card (Form I-551)
• Employment Authorization Card (Form I-766)
• Employment Authorization Card (Form I-688A)
• Employment Authorization Card (Form I-688B)
• Temporary Resident Card (Form I-688)
 
Non-Acceptable:
 
• Mexican Voter Registration Card “Credencial Para Votar”
• Permanent Resident Card (Form I-551 does not have a visible signature)
• Alien Registration Receipt Card Form I-151 (was replaced by the I-551)
• USA B1/B2 Visa/BCC (Form DSP-150)
• Non-resident Border Crosser Card (Form I-586)
• Non-resident Alien Mexican Border Crosser Card (Form I-186)
• Non-resident Alien Canadian Border Crosser Card (Form I-185)
• U.S. Citizen Identification Card (Form I-197)
• Arizona Department of Corrections Released Offender Card
• D.E.S. Food Stamp Card
• School Identification Card
• Tribal Identification Cards that DO NOT contain bearer’s signature
