Death Certificates

The Mohave County Department of Public Health registers deaths that occur in Mohave County. Death records are available through an online death registry system that is maintained by the Arizona Department of Health Services, Office of Vital Records.

Certified Copies

Who Can Obtain a Death Certificate
Only persons 18 years of age or older may obtain a certified copy of a death certificate.

Certified copies of death certificates are available to immediate family members or those with a documented legal interest. To obtain certified copies, you must complete an application, present proof of relationship or current legal interest, present acceptable identification, and submit payment.

Debit/credit cards will have a processing fee of a minimum $1.50 or 2.39%. You may apply for certified copies in person or through the mail.

Certified copies are $20.00 per copy and we accept debit/credit card, money order, cashiers check or exact cash. We DO NOT accept personal checks.

Effective 1/1/2019, the state FEE for death records will be:

Certified Death Certificate..................................$20.00
Certified Delayed Death Certificate....................$20.00
Amendment of Certified Death Certificate..........$30.00
Non-certified Copy..............................................$ 5.00

Arizona Vital Records has death records ONLY for deaths that occurred in Arizona. If you need a certified copy of a death certificate from another state, please visit the CDC for more information.
 

Note: An applicant presenting documents from a foreign country as proof of relationship or proof of legal interest must provide certified copies of the documents. Photocopies are not acceptable. The applicant must also provide a certified translation of the documents.

Arizona is a "closed record" state. That means that vital records are not public record. Arizona law restricts the public's access to vital records to protect the confidentiality rights of our citizens. Arizona Administrative Code R9-19-314 & R9-19-315 specifies that only the following persons may receive a certified copy of a death certificate:

Instructions for Applying by Dropbox or by Mail

Applications Submitted via Dropbox

Each location has a secured dropbox. When submitting an application via dropbox, please ensure you have the following:

• A copy of your valid government-issued picture ID which bears your signature.
• Proof of eligibility. More information at AZDHS.
• Appropriate fee
• If possible, complete and print the Application Form and bring it with you.
(A copy of the application is also available in the Vital Records office.)

Applications Submitted Through the Mail

• A completed Application Form.
• A copy of your valid government-issued picture ID which has your signature. Or, have your signature notarized.
• Proof of eligibility. More information at AZDHS.

Acceptable and Non-Acceptable Identification for Requesting Death Records

Acceptable:

• United States issued Drivers Licenses
• United States issued Identification Card
• United States Territories Drivers License or Identification Card
• Tribal Identification Cards that contains bearer’s signature
• U.S. Military ID Cards that contains bearer’s signature (some do not)
• Passports-United States or Foreign issued
• Visa –In a passport and the bearer’s signature is on the passport not the Visa
• Foreign Consulate Identification Card
• Resident Alien Card (Form I-551)
• Employment Authorization Card (Form I-766)
• Employment Authorization Card (Form I-688A)
• Employment Authorization Card (Form I-688B)
• Temporary Resident Card (Form I-688)

Non-Acceptable:

• Mexican Voter Registration Card “Credencial Para Votar”
• Permanent Resident Card (Form I-551 does not have a visible signature)
• Alien Registration Receipt Card Form I-151 (was replaced by the I-551)
• USA B1/B2 Visa/BCC (Form DSP-150)
• Non-resident Border Crosser Card (Form I-586)
• Non-resident Alien Mexican Border Crosser Card (Form I-186)
• Non-resident Alien Canadian Border Crosser Card (Form I-185)
• U.S. Citizen Identification Card (Form I-197)
• Arizona Department of Corrections Released Offender Card

Requests for Fetal Death Certificates

Only persons 18 years of age or older may obtain a certified copy of fetal death certificate. In addition, to protect the confidentiality rights of our citizens, Arizona Administrative Code A.A.C.R9-19-317 specifies that only the parents of a fetus or another person authorized by one of the parents may obtain a certified copy of a fetal death certificate.

If you are not one of the parents of the fetus, you must provide written authorization from one or both of the parents in order to receive a certified copy of the fetal death certificate, and a valid government issued picture I.D. in order to obtain a certified copy.

A certified copy of a fetal death certificate may be issued only to the parents of a fetus or to another person upon authorization from a parent if all of the following criteria are met:

1. The applicant is named as parent on the certificate.
2. The applicant submits a letter signed by one or both of the parents that authorizes the applicant to apply and receive a certified copy of the fetal death certificate and/or the certificate resulting in still birth. The letter must be signed by one or both parents in the presence of a notary or the parents(s) must provide a photocopy of their valid government issued identification. The applicant submits a signed application.
3. The applicant provides valid government issued identification or notarized signature on the application.
4. The applicant submits the appropriate fee(s).
