Home Birth
Arizona Revised Statutes §§ 36-333 and 36-333.01 and Arizona Administrative Code R9-19-203 and R9-204 provide requirements for the registration of births that did not occur in a hospital.
Home births in Mohave County must be registered with the Mohave County Department of Public Health using the Certificate of live birth instruction guide and Certificate of live birth worksheet.
If the County Vital Records or the Bureau of Vital Records determines a request submitted to register a birth record is insufficient, a written notification will be submitted to the person who submitted the request specifying the issue and the procedures required to resolve the issue.
